It is possible, however the freezing point of "dry ice" or frozen CO2 is very low. Although it is true that it will sublime, even in an enclosed arena, suffocation is not the main deleterious factor in not using it. Rather, the temperature is the main factor. If you have any experience with frozen CO2, you would know that extended contact with it can lead to frostbite. It would not be nice if people who fell ended up getting frost related injuries. Additionally it would be very expensive to maintain since the stable temperature for a solid state of CO2 is much further from the atmospheric temperature than that of ice. So basically safety and cost are the main reasons ice is generally used.
It is possible and there are artificial ice rinks mostly made of plastic but just like artificial ski slopes, nothing beats the real thing.

No, as the other chemicals will be toxious and aren't really suitable for health reasons. So water is the most suitable as it doesn't harm the environment or you.
In principle it is possible, but you will have to carry your own scuba tank and mask with you, since as dry ice sublimes it will form gaseous CO2 and if the "dry ice rink" is enclosed, all the customers will suffocate.
