Due to operator error, a power excursion occurred at one of them, causing an explosion and fire that spread radioactive contamination throughout the area. The fire was eventually extinguished, and a coffin constructed about the damaged reactor to contain its radiation. Something over forty people eventually died from radiation poisoning.
