There are sensors for voltage, current, frequency and phase in each leg of each multiphase feed. Those sensors convert the values to an analog or digital equivalent of the monitored info. If analog they are wired into analog inputs of PLCs to gather the info and alarm conditions for SCADA. If they are digital they put the info into a multiword message (in whichever protocol the PLCs speak to SCADA). Their precision is good enough for exact monitoring of the power system and for calculation of the efficiency of the system
