Heat transfer is the study of how energy actually flows through systems by conduction, convection, etc.

Thermo is the study of the basic laws that govern heat transfer.
Thermodynamics is the study of the flow of heat , heat and work and has three laws governing it.
Heat transfer can follow thermodynamic rules not the other way.
Heat transfer is the movement of heat energy by Conduction, Convection and Radiation between hot substances and cold substances within a controlled system as in 'Heat Exchangers' for instance.
The process is governed by the laws of 'Thermodynamics' which deals with the effects of Work, Heat, Energy and Entropy within a system .
