For menstrual pads, there are patterns out there on the internet for making menstrual pads out of cloth. Once they are used, they are laundered just like any other article of clothing. What do you think they wore in the old days before disposable pads and diapers were invented? Where do you think the term "rag" comes from when referring to a menstrual pad?

Cloth diapers are cheaper in the long run than disposables, true, there is a larger cost up front but each time you use those cloth diapers they become cheaper and cheaper until the are less than the disposables. The same would hold true for menstrual pads, but pads being smaller can be made out of scraps of material which would help keep cost down.

Cloth diapers breathe where disposables won't. Even with wearing diaper covers over the cloth diapers, they breathe better. This means next to no diaper rashes. You just need to make sure that you change them right away. They are better for your baby's skin. Again, this would hold true for the cloth menstrual pads. They would be softer and more comfortable against a woman's vagina.

I think your question is an interesting one. As you're well aware, our language matters, and that it isn't about us and them or what environmentalists do or don't do. It's about all of us coming up with solutions together.
Many don't. Many use biodegradable tampons or pads, some use the menstrual cups which are reusable/washable.

As for diapers, many people opt for cloth which is still a debate since they use resources for the constant washing (assuming a wash in hot water). Other's use diapers made in a more natural way without some of the chemicals that are bad for the earth.

there is a thing called a diva cup that some people use instead of a pad/tampon.
and actually there is a group of parents that dont use diapers on their children.

My kid plays outside in a fenced yard. I hose him off when he comes inside.
you can get a type of sea weed that you use as a reuseable tampon that you just have to wash. use that or a slab of sod.
The best solution for diapers is Diaper Free. There's a great book on the topic by Ingrid Bauer. Think about it--diapers are a very new invention in the history of the world, and they still haven't been adopted by all cultures around the planet (thank goodness). If infants and their parents are trained how to live without diapers from day one, they don't need to go through 1-4 years of uncomfortable messy, stinky diapers, cloth or disposable.
