If you mean that the opposing listener is thinking about your question and has understood, then I think the body language should tell you everything. For instance, does he twitch and look around while you're talking, or is his gaze too intent (i.e. day-dreaming)...However, the true test is the response, but you can judge whether or not a person has the capacity to think and understand what you're trying to convey when you first begin the conversation.
