I really need help - With my mind!?
Answer:
I would go see a psychologist or a general practitioner. I wish you the best.
You need to see a doctor.
It sounds as if you are suffering from extreme anxiety. Perhaps you have exams coming? Or maybe you are being bullied in some way? Do you feel that you are not able to achieve what you want and that you are being pressurised? There are many reasons for the behaviour you are experiencing and there must be a trigger for all of this. Only you know what it is that is bothering you, it may be something very personal that you can not speak to your friends or family about. In that case, maybe you should seek some professional help. I hope you manage to find a solution. Good luck! :)

Take St. Johns Wart. It is suppossed to make your head a clearer thinker.
If I were Freud, I would tell you that you have a deeply troubling and repressed conflict somewhere in your unconscious that is preoccupying your brain and detracting from all of your other abilities and absolutely trying to destroy your self-efficacy.
For once, I actually think that Freud might have something here. Do you have a conflict or anything going on? It doesn't have to be something completely devastating, like the death of a parent or a recent traumatic incident. Perhaps your life has felt a little stagnant lately, and you're just very unsatisfied with how things are and this is frustrating you.
If that is the case, finding something that really gets your mind involved may help you. Many professionals' jobs are monotonous and boring, requiring little intellectual involvement. Entertaining your brain will make it happy! It can be something as simple as maybe thoughtfully answering people's questions on FunQA.com! This stimulation should also help those feelings of helpless confusion in your dreams. Though I haven't read The Interpretation of Dreams (yet! I'm working on Carl Jung's book...), I'm pretty sure that Freud would attribute those to your unconscious conflict too.
As for seeing a psychiatrist/psychologist, this can be helpful if you really let yourself discuss openly with them. They should be able to ask the right questions, guide the direction of the conversation, and probably pinpoint your problem. If anything, it'll feel good just to let it all out to somebody who will listen.
