Oh thats such a difficult subject. Since I personally had that to contend with there would be various reasons to look over.
-What do the Drs say, most importantly?
-In a coma, then who will step up and be their trustee and take care of business for them with their best interest at heart
-Who can be trusted to do such a job?
I believe its a combo of all. You all need to sit down together and discuss, not argue. List out the pros and cons.
Be realistic, what would that person have wanted?

After the horrible thing that happened to Terri Schiavo, that would be hard to say.
I would say family/spouse unless there is conflict with them, because they usually know the person better than anyone else. I would trust my family in that situation.
In the case of the terminal patient it should be up to the patient because we all have the right to deny medication or any medical treatment. No one should have the right to choose for you if you can make the choice yourself.
In the case of someone who is comatose if there is no living will or written expressed wishes it should go to the husband or wife. If the person isn't married it should go to next of kin if there is no next of kin then it would be up to the lawyer if there was one.
I believe it should be a family issue and not an attorney cause he knows not the feeling of family and with much emotions it should be left up to the family.
