Why is it so hard not to laugh at somebody when they fall over?

No, it's not just you, I think most people have that reaction. I know the biggest argument my parents ever had was when my mother fell through the barn floor, caught herself with her arms and had to be hauled out by my father, who laughed at her. She was angry for a week.
Most people, when they take a pratfall, laugh along with everyone else. Of course, if they are lying on the ground in a coma with serious injuries, it's not so funny.
Maybe this explains why America's Funniest People has stayed on the air so long. It is just an hourlong series of pratfalls.
As long as you feel bad about it afterwards, it's ok.

I think for most people its funny because it reminds them of when they did it. It was embarrasing when it happend to you for about 20 minutes then its hilarious.

I was looking into the street from my office window yesterday and saw some fat dude fall into a 3 ft hole that was the result of on going construction...I laughed my as s off.

I do it too. Some people say it is a nervous reaction, but I think it is plain funny. But I think it is a nervous reaction. yeah.
Takeaway the thought that falling over is an embarrassment (though it is) and you'll be able to stop yourself from laughing the next time.
When such thing happens, I simply shift my eyes to something totally different, at a different direction, and focus on whatever as long as it's not the fall... that way I can stop myself from reacting in what-so-ever way.
Or you can run to help and say "are you ok"?
Everyone laughs when someone falls over. That happens because is a strange situation, something you weren't expecting to happen. Also, after seeing how the victim is embarrassed, is hard not to laugh!
For me it has a lot to do with the context of the fall. If an old lady falls while trying to gte out of a car I do not laugh, but if a friend of mine falls while doing something stupid I do.
Laughing has a lot to do with the unexpectedness of a fall. Every day is very routine for most of us, and if something happens that is different it may warrant different reactions such as laughing or concern.
Just be there to help the person get up, and you can react any way you want to.

i know this sounds REALLY mean.. pls dont give me thumbs down..i was young.. but when i was like 13 we were driving down the street and this kinda big lady tripped and almost fell, she was waving her arms all about so she wouldnt fall.. and i rolled down my window and started going HAHAHAHAHA... then I rolled my window up.. but still found it to be quite funny so I rolled my window back down and laughed again..
No, it's just plain hilarious. I laughed my *** of when a "friend" of mine fell and broke his collar bone and was hospitalized for a week or so.
