Approaching death which is unknown to them of what they will face.
Some people do not want to die in pain. Most people want to pass peacefully and with dignity. Possibly a combination of fear of pain and the unknown occurrences after death.
A lost of control. Control of their lives, their health, Lose of control over how they will live their lives for the rest of their lives. Because when you're that sick the illness dictates how you will live your life to a large extent. Lose of the contact with their loved ones. Lose of control over a large part of their medical treatment. Lose of who they are as they used to know themselves. Terminal illness changes the fundamental way that you relate to the world and how the world relates to you. This will affect you in very intimate ways not only medically but emotionally. How do you now change your relationship to everyone else?? They will now change how they relate to you and you to them, You are no longer a well person with all their faculties you are than perceived to be a sick person who may or may not have all of their faculties.
