I haven't pierced my nipples before but it is the same as other surface piercings which I have done myself. I took a safety pin (not a huge one- it was about the size of your top finger joint because the pin wasn't as wide) and stuck it through my skin on my forearm, hand, and finger webbings, and ears, about 10 in all. It doesn't really hurt much as long as you approach it calmly. Make sure you have nipple rings to stick in after you pierce it though because safety pins tend to catch on things. sometimes I had to keep the safety pin in a few days to let it heal before sticking a wider earring/barbell in.
