What is the best way to earn self respect and confidence?

By being rigorously honest with your own self. Live and let live as far as others are concerned.
self esteem is built up from the inside out not the outside in. make a list of your qualities, things you like, things you are good at doing, also list what others call faults and find what you use and keep them... for example. I'm called stubborn by other, but it's actually determination I used that in my life so even if its a fault in the eyes of someone else it's good for me. Be as honest as you can... there are faults like I use to stutter, or I use to lose my temper... things like these you work at ... until you are the person you like being and the person you want to be... nurture yourself... mind over mater means working with nature, not against it.especially when it's your own nature . the goal is not to be perfect but to be perfectly you.
by doing the things your good at and accomplishing goals. thinking positively and faking it till you make it (imagine you are all ready good at what you want to be)
self respect is not earned, it is learned. it is not something that someone can give you. your self-respect comes from experiences you've had, your moral character, how you were raised/nurtured as a child, what you accept or do not accept. it comes from what you'd do if no one was looking. it comes from how you feel about what you've done or allowed others to do to you. your self-respect can be seen by others in the things you do and say. your self-respect can be seen in the way others see you.

confidence can be learned and earned. your belief in your abilities to achieve build your confidence. doing something that you never thought you could, and whether or not you succeeded, you got past it and felt great/relieved for your efforts. people wear a certain look which they believe boosts their confidence. its nothing far-fetched or magical. people's confidence in others comes from experience in trusting the other individual's abilities to act in various situations.
Set a goal for yourself, kind of like a New Years resolution, and DO IT!

It could be as simple as baking a pie for the first time without any help or reading a difficult to understand book, that you know will benefit you from cover to cover.

Distance yourself from anything and anybody that you know might be harmful to you. {smoking,drugs, etc.}

Take a long look at yourself in a mirror and learn to like {and over time try to improve} on what you see.

Be proud and accept who you are!

I say a little mantra to myself over and over, whenever I feel especially glum:

Think Positive, Act Positive, Be Positive!
