A cluttered environment comes from a cluttered mind. I know. While never diagnosed, I believe I have ADD (and my brother was just recently diagnosed). I've been one of the most untidy and cluttered people I know and it took a lot of time to come to peace with that even though I've improved a lot (though I still have my moments of impatience with it). I realized that clutter springs from not making clear decisions on where to specifically house everything and not giving yourself the time to organize and tidy up.
This is a great website from which I got a ton of helpful ideas and a forum of supportive people: www.getorganizednow.com. And a lot of the questions you asked have already been directly answered at this website.
Now, to answer your questions more directly...
1. I've learned to ask myself what is important and not important to keep. If I haven't used it, looked at it, or just plain appreciated it for a long time, then I get rid of it.
2. The strangest thing? Not going there. :-)
3. I would save my pictures from a fire. But, even that I know I could live without as I had a ton of pictures on my hard drive and that crashed a few months ago. I cried. But I'm still here taking new pictures.
4. I don't fall for buying a lot of stuff now. I know it just becomes a hassle to maintain them if I know I don't need or really want them.
5. When someone offers me their hand-me-downs I've learned to say 'No. Thank you.' And when they give me that shocked look like, 'You don't want this crap I don't want that I'm trying to push off onto you??!!' I explain that I have almost the same item at home or that I know I'll never put to use whatever they are trying to give me.
6. Holidays and Bdays... You could make it known to family and friends that you like gift cards or they can give you a gift that you can send to your favorite charity.
7. Do I like living in clutter? Well. I have a high tolerance for it. LOL. It took me a long time to realize that while my clutter is part of who I am it is not a detriment to my self-worth. I explain to people very frankly that I'm messy. While I'm great at other things, this is just one of those things that I'm not great at keeping up with consistently.
8. If I see I'm not using a broken item, I tend to get rid of it.
9. I don't see myself as getting rid of all my clutter, though I am a lot better at keeping it organized. It doesn't keep me from enjoying my life and I see it as one of the idiosyncracies that makes me who I am.

I guess Im a pretty practical person in the way that if something I own is not usefull, I actually love throwing it out! I take my clothes to consignment stores, and everything else goes to charities. Ive also moved around from city to city my whole adult life, so I think this fact makes me feel bogged down when I own too much stuff. I dont want to feel tied down by anything. I do collect small antiques and nick nacks, but the whole collection can fit in the back of my SUV. The reason I keep them is that they are either rare and beautiful to me, or sentimental.
The only things I own that are extremely important are 2 of my Moms paintings, and this wonderful Mac ibook Im typing on right now (mainly because it contains my entire rare funk and soul collection). I actually feel a strange sense of satisfaction when I can throw away just about anything, even as small as shampoo containers...lol!
