Heart attack - in real life, if you jumped off a building, you would most probably die from a heart atack in mid-air. In a dream, you obviously think it's real.

It is possible I suppose to have a dream that raises the heart rate up to a point that tears lose a clot. Thereby, leading to a stroke or heart attack. But any source of heart rate increase would have done it. So I am not sure you could say that the dream caused death.
Yes, u can die of a stroke bcoz of a shocking dream. This is more common for old pple w/ ill health. Or in very rare cases for women (documented), sleeping in carbo-over-load, as in w/ a full stomach, can cause toxic release of the pancreas while dreaming, slowly killing the person by morning. (This happens to Asian men due to their specific diet practices)
